I am providing an example of CYP2C19 and drug A
For example, assuming that CYP2C19 influences the metabolism of drug (A) into inactive drug. Then CYP2C19 *1/*2 (intermediate metabolizer, IM), CYP2C19 *2/*2 (poor metabolizers, PM) will be at risk of developing high plasma levels of drug (A). Therefore, patients with intermediate and poor metabolizer phenotypes are at high risk for developing adverse events from Drug A.

The actionable genotypes/phenotypes for Drug A: are CYP2C19 genotypes *1/*2 (IM), *2/*2 (PM).
Rationale: Since CYP2C19 influences the drug metabolism of drug A, individuals with intermediate and poor metabolizer phenotypes are likely to have high drug levels compared to these individuals with normal metabolizer phenotype, thus, these patients are at risk of developing side effects from drug A

A man’s scalp is torn open when the metal stairs of his recreational vehicle fall on his head. He is immediately taken to the emergency department, where a CT scan comes back normal and no skull damage is found. The care team turns their attention to the wound, but the anesthetic doesn’t appear to be working. A nurse shares the case on Figure 1, explaining the dilemma:

An emergency physician gently questions the nurse:

A neurologist recommends lidocaine with epinephrine, a vasoconstrictor that one nurse refers to as “the chemical tourniquet.” Several other healthcare professionals suggest other anesthetics.

– Biomarker is where on the gene. For Example, if they store genes, they can find where in the gene mutations lives and where is found.
– Personalized Medicine: is based on genetics and biomarkers. So basically doing is personalizing your medical care /medications to that specific patient So what is happening is there is something making that person different than the majority of people and what happens it is usually genetics.
– Now a redheaded person has two recessive genes so for example each parent, has a dominant brown hair gene but they also carry a recessive gene from both parents so one in four children can come out redhead and that’s why there are not so many redheads around because it is a recessive trait And somewhere on the genetic makeup of a redhead is a biomarker that is doing something to cause them to be resistant to a normal dosage medication, for example, Lidocaine ( topical local anesthetic).
-The comment that the nurse made was knocking the patient out.
– If you look at lidocaine or any caine will do and what anesthesia will do will basically be a type of numbing medicine so somewhere in the genetic makeup of a red-headed person, there is a mutation or biomarker that is changing the ability to respond to a normal dose of that medication.
– So the nurse saying that had a 8 yo patient had enough to knock off a wrestler basically is she saying, she is supporting the fact that yeah, I think this is a true statement because I had 8 yo redhead kid and had to give a lot of anesthesia to knock her out. So it is a comparison—All redheads are resistant to certain medications.
– How do you feel about the importance in the study of genetics for a nurse? It is very to important for a nurse because nurse have to be safe in taking care of a patient. Nursing is evidence- base practice which consist of research, identifying and evaluating the most recent and reliable medical journals/ sources and learn how to connect the genetic, pathophysiology, findings and the medicine and make correct medical decisions. so we need to do our research and learn what we learn and get the knowledge and know the research, the science behind it, and connect everything before touching our patients. And who’s to say these patients are a fluke and they coincidentally just happened to be redheads?

This is how experience nurses come in. They have a lot different things happening in their career. Having the knowledge and the experience what things will work than the student nurse because they haven’t had the experience yet. As through experience, you will say this happened to me before and this is what worked for that circumstance and might work here or might work from before.
– When patient comes in what that thing, you can say hay this might be a genetic thing you may need to evaluate a little bit more or handle to situation a little bit differently.
– Lidocaine is there a maximum dose? Injectable or topical? If there is a maximum dose, you want to address that because then what would be the next step be? In the case study Example, the doctor with general anesthesia. Is there a maximum dosage to give with general anesthesia.
